PLAYING WITH DATA SURVEY CONSENT FORM
ABOUT PLAYING WITH DATA: Playing with Data is a three-year, National Science Foundation-funded research and development project being conducted by Education Development Center’s Center for Children and Technology (EDC| CCT) and by GlassLab, a developer of educational digital games. The goal of this work is to investigate what kinds of tools and information support middle-school science teachers in using data from students' digital gameplay to guide their instruction.
SURVEY: This survey asks questions about your teaching context, school climate, and how you use digital games in your teaching to support formative assessment. It should take about 20 minutes to complete. Your participation in this survey is voluntary, and you may withdraw at any time without consequence. There are no anticipated risks to completing this survey beyond those encountered in everyday life.
CONFIDENTIALITY: All information you provide in this survey will remain strictly confidential. Your responses will be used only for the purposes of this study, and will not be disclosed or used in any identifiable form for any other purpose. To protect your privacy, your responses will be downloaded as encrypted data, and the resulting files will be stored behind a firewall in a password-protected document library. Findings from this survey will be reported only in statistical summaries, and individuals will not be identified.
